This has got to be a dream/hallucination sequence.
It's got to be another dream sequence, remeber the last time Locke went into the jungle, there were loads of dream moments. But the fact that Boone's pushing the wheelchair ties the scene to "Deus Ex Machina", remember that Boone more or less carried Locke to the plane site? Maybe Locke's realising how much he needs Boone if he's going to continue his stay on the island? Maybe?
And as far as the Locke and Hurley scene in the jungle is concerned, the presence of the special FX truck almost guarantees a monster scene. What else could it be?
Very good point about the FX Truck and the monster. That confirms some other spoilers I've been told about Episode 2 as well.

man im tired of all these original character flashbacks - lets get some fenry, zeke, rousseu, walt flashbacks going
definitely, the connection between the hurley and locke trailers, plus the special effects bus could possibly mean the deck that hugo destroyed was also the cause of locke's paralysis. thus needing some special effects for the destruction.:)
All the original characters are having one last flashback this season to finish up all the loose ends, then they'll be diving into all the new characters.
They said they'll LIKELY get to rousseau in season 4. So you'll probably have to wait a year for that.

Maybe the supposed "jump the shark" moment is that somehow the flashbacks we thought were real change...

The guy pushing the wheelchair doesn't look like Boone to me, we can't see his face either! Is there any proof that it is Boone?

Sally, the Boone reference came from another spoiler on the site in which someone from Hawaii saw Boone pushing Locke.
M-Slice: The Don't Trust Sarah quote was mentioned in a previous spoiler. I agree it sounds likea the little girl saw Sarah with another man.
YES! Boone/Locke reunion! This'll be a great episode. As far as the special effects? maybe the polar bear will make its appearance? And I agree, I want some Walt flashbacks!
